When to get tested
- After 12 months of trying, or after 6 months if the woman is 35 or older
- Straight away if periods are irregular or absent
- If there is known endometriosis, PCOS, previous pelvic surgery or infection
- If the man has had testicular problems, surgery or cancer treatment
- After two or more miscarriages
Tests for women
- AMH blood test
Estimates egg reserve, the number of eggs remaining. It can be done on any day of the cycle. - Transvaginal ultrasound and antral follicle count
Checks the uterus and ovaries and counts small follicles, usually early in the cycle. - Hormone tests
FSH, LH and oestradiol early in the cycle, with thyroid (TSH) and prolactin. - Tubal test (HSG or sono-HSG)
Shows whether the fallopian tubes are open. It is done after the period ends and before ovulation. - Follicle tracking (follicular study)
A series of scans to see whether, and when, an egg is released. - Hysteroscopy or laparoscopy
Only when other tests suggest a problem inside the uterus or pelvis.
Tests for men
- Semen analysis
Measures count, movement and shape of sperm, after 2 to 7 days without ejaculation. - Hormone tests and ultrasound
When the semen analysis is abnormal.
Tests before IVF or IUI
Before assisted treatment both partners are also screened for infections such as HIV, hepatitis B and C and syphilis, and the woman's blood group, blood count, sugar and thyroid are checked. Your doctor will tell you if any test needs fasting.
What happens after the tests
Your doctor goes through the results with you, explains what they mean together, and recommends the simplest treatment likely to work. That may be timed intercourse, ovulation induction, IUI, IVF or ICSI, or sometimes simply more time.
Questions about fertility tests
Which fertility tests should we do first?
For most couples: an ultrasound and AMH test for the woman, a semen analysis for the man, and a test of the fallopian tubes. These three answer the main questions about eggs, sperm and the route between them.
What is a normal AMH level?
AMH falls naturally with age, so a normal level depends on how old you are. A low AMH means fewer eggs remain but does not mean you cannot conceive. Your doctor interprets it with your age and ultrasound.
Is the HSG test painful?
Most women feel period-like cramps for a few minutes. Taking a painkiller beforehand helps, and you can go home soon after.
Can both partners be tested on the same day?
Often, yes. The semen analysis, blood tests and ultrasound can usually be arranged on one visit, with the tubal test planned for the right day of the cycle.
Do we need tests if we already have reports from another clinic?
Bring them. Recent, reliable results usually do not need repeating, which saves time and money.
