What is repeated IVF failure?

There is no single agreed definition. It is usually considered when several good-quality embryos have been transferred, over two or more cycles, without a pregnancy. The woman's age and the number and quality of embryos matter when deciding what counts as unexpected.

Common reasons IVF does not work

  • Embryo factors: chromosomal abnormalities in the embryo are the most common reason an embryo does not implant, and they become more frequent as women get older
  • Uterine factors: polyps, fibroids, scar tissue, a uterine septum or a thin lining
  • Hydrosalpinx: a blocked, fluid-filled fallopian tube that can leak fluid into the uterus
  • Sperm factors: for example high sperm DNA fragmentation
  • Stimulation or lab factors: a protocol that did not suit your ovaries, or embryos transferred at an earlier stage
  • Other health factors: thyroid problems, uncontrolled diabetes, smoking or a high body weight

How we investigate

  1. Review of previous cycles
    Bring every report you have: stimulation charts, egg and embryo reports and transfer details.
  2. Hysteroscopy
    A direct look inside the uterus to find and treat polyps, adhesions or a septum.
  3. Ultrasound and tube assessment
    To check for fibroids or a hydrosalpinx.
  4. Tests for both partners
    Chosen for your history: for example thyroid and sugar tests, karyotyping, and sperm DNA fragmentation.

What can be done differently

  • A stimulation protocol adjusted to how your ovaries responded before
  • Growing embryos to the blastocyst stage to choose the strongest one
  • Genetic testing of embryos (PGT-A) in selected couples
  • Correcting problems in the uterus or tubes before the next transfer
  • A frozen embryo transfer with a carefully prepared lining
  • Honest discussion of options such as donor eggs when they are the realistic path

Some tests and "add-on" treatments offered for IVF failure have limited scientific evidence. We explain what is proven and what is not, so that you do not spend on treatments unlikely to help.

Questions about repeated IVF failure

How many failed IVF cycles count as repeated failure?

There is no fixed number. Doctors usually consider it when two or more transfers of good-quality embryos have not led to a pregnancy, taking the woman's age into account.

Should we take a second opinion after a failed IVF cycle?

A second opinion is reasonable after one or more unexplained failures. Bring all your previous reports so the review can be detailed.

Will genetic testing of embryos (PGT-A) help?

It can help some couples, especially older women or those with many embryos, by avoiding the transfer of chromosomally abnormal embryos. It is not needed for everyone, and your doctor will explain whether it suits you.

Can the uterus be the reason IVF failed?

Yes. Polyps, fibroids, scar tissue, a septum or a fluid-filled tube can all lower the chance of implantation. Many of these can be treated with hysteroscopy or laparoscopy.

How long should we wait before trying again?

Many couples can start the next cycle after one or two periods. If tests or surgery are needed first, your doctor will plan the timing with you.
