Overview

At Ishwa IVF in Jaipur we look for a cause, treat what can be treated and support you through the next pregnancy. Even when no cause is found, most couples go on to have a successful pregnancy.

What is recurrent miscarriage?

Recurrent miscarriage usually means two or more pregnancy losses. Investigation is recommended after two losses, especially for women over 35 or couples who have struggled to conceive.

Possible causes

  • Chromosomal problems in the pregnancy, the most common reason for a single miscarriage, and more frequent with age
  • A balanced chromosome change in one parent, found in a small number of couples
  • Differences in the shape of the uterus, such as a septum, as well as fibroids or polyps
  • Antiphospholipid syndrome, an immune condition that causes blood clotting in the placenta
  • Thyroid disease and uncontrolled diabetes
  • Lifestyle factors such as smoking and a high body weight

In about half of couples no cause is found. This is called unexplained recurrent miscarriage, and the outlook is still good with supportive care.

Tests we recommend

  1. Ultrasound of the uterus, sometimes 3D, and hysteroscopy if a problem is suspected
  2. Antiphospholipid antibody tests, repeated after 12 weeks if positive
  3. Thyroid and blood sugar tests
  4. Chromosome tests (karyotype) for both partners, and of the pregnancy tissue when possible

Treatment

  • Hysteroscopic surgery to remove a uterine septum, polyps or fibroids inside the cavity
  • Low-dose aspirin and heparin during pregnancy for women with antiphospholipid syndrome
  • Correcting thyroid or sugar problems before the next pregnancy
  • Progesterone in early pregnancy for some women with bleeding and a previous miscarriage
  • IVF with genetic testing of embryos (PGT) for couples who carry a chromosome change
  • Early pregnancy care: early scans and regular support, which helps couples through the anxious first weeks

Questions about recurrent miscarriage

When should we get tested after a miscarriage?

Testing is recommended after two pregnancy losses. If you are over 35 or have had difficulty conceiving, it is reasonable to seek advice after one.

Is a miscarriage caused by something I did?

Almost never. Most miscarriages happen because of chromosomal problems in the pregnancy that nothing could have prevented. Normal work, exercise and intercourse do not cause miscarriage.

What if all our tests are normal?

This is common and is called unexplained recurrent miscarriage. The chance of a successful next pregnancy is still good, and early scans and regular support in early pregnancy help.

Can IVF prevent miscarriage?

IVF alone does not prevent miscarriage. When a parent carries a balanced chromosome change, IVF with genetic testing of embryos can help select embryos without the imbalance.

Do blood thinners help everyone with recurrent miscarriage?

No. Aspirin and heparin are recommended for women with antiphospholipid syndrome. They are not advised routinely for unexplained miscarriage.
